Metabolically Directed Approach to Hormone Replacement Therapy (HRT)

  • When replacing sex and/or adrenal hormones, the patients overall metabolic balance must be taken into account for optimal results 
  • Look for the Root Cause of the Hormone imbalance – dietary indiscretions, chronic stress, thyroid problems, medications, nutritional supplements, chronic inflammation

Example of Metabolically Directed HRT in Estrogen Dominance

  • In a post-menopausal female – that is dysaerobic, progesterone replacement is extremely beneficial initially as progesterone is anti-dysaerobic. However, the use of calcium D glucurate should be started after the dysaerobic imbalance has been addressed as calcium d glucurate is anti-anaerobic 
  • Conversely in an anaerobic patient, the anaerobic imbalance should be treated prior to adding the progesterone, whereas calcium D glucurate can be utilized in the beginning of treatment

Another Example of Metabolically Directed HRT in Estrogen Dominance

  • In a post-menopausal female – that is parasympathetic   progesterone replacement is extremely beneficial initially as progesterone is anti-parasympathetic. Additionally, the use of calcium D glucurate should be started initially as well because reducing estrogen will also reduce parasympathetic imbalance 
  • Conversely in a sympathetic patient, progesterone replacement and estrogen reduction (both of which tend to stimulate sympathetic nervous system activity) should be deferred until the sympathetic imbalance is better controlled

Example of Metabolically Directed HRT in Testosterone Deficiency

  • In a dysaerobic male, testosterone replacement should begin only after the dysaerobic imbalance has been addressed, otherwise, the additional testosterone will push the patient further into the dysaerobic imbalance   
  • Conversely in an anaerobic male patient, testosterone replacement will help reduce the anerobic imbalance and can be started early in the treatment process

Metabolically Directed HRT

  • Measure hormone levels
  • Measure body chemistry (acid-base, autonomic balance, anabolic-catabolic balance)
  • Administer HRT while also accounting for changes in body chemistry 
  • Consider and treat the effects of chronic stress and inflammation on hormone levels
